Famicom Disc System instruction manuel dimensions?

Does anybody know the correct dimensions/layout for the Famicom Disc System instruction manuels?  I'm talking about the ones that came boxed wuth games like Metroid and Legend of Zelda such as this:

I'm looking to make my own custom Famicom Disc System manuel with the same format and dimensions.
